The percentage of workers in high deductible health plans participating in health savings accounts, health reimbursement arrangements, and flexible spending accounts.

Outpatient prescription drug benefits: Type of coverage
The percentage of workers enrolled in outpatient prescription drug plans that provide coverage for generic drugs, brand-name drugs, mail order drugs, and formulary drugs. Formulary drugs are drugs approved by the healthcare provider. Drugs not approved by the healthcare provider are non-formulary drugs, for which enrollees receive less generous benefits, such as a higher per-prescription copayment.

Outpatient prescription drug benefits: Copayment provisions
The percentage of workers enrolled in outpatient prescription drug plans that require a copayment for generic drugs and brand-name drugs, and the median copayment amount per prescription.

Basic provisions - Retirement

Defined benefit plans: Eligibility requirements
The percentage of workers participating in plans open to new employees with minimum age or service requirement, median age requirement, and median service requirement (in months); and plans not open to new employees.

Defined benefit plans: Plan sponsor
The percentage of workers participating in plans sponsored by a single employer, multiple employers, or other sponsor.

Defined benefit plans: Primary formula
The percentage of workers participating in plans with traditional formulas, including percent of terminal earnings, percent of career earnings, dollar times years, and percent of employer contribution; and non-traditional formulas, including cash balance, pension equity, and other formulas.

Defined contribution plans: Type of plan
The percentage of workers participating in savings and thrift, deferred profit sharing, employee stock ownership, money purchase pension, simplified employee pension (SEP), savings incentive match plan (SIMPLE), and other types of plans.

Savings and thrift plans: Summary of provisions
The percentage of workers participating in plans with pre-tax contribution, post-tax contribution, automatic enrollment provision, automatic escalation feature, and employee choice of investment for employee and employer funds.

The percentage of workers participating in plans with a maximum employee contribution based on a percent of earnings (up to the Internal Revenue Code limit) and the maximum percent of earnings amount by selected percentiles; a contribution of any amount up to the Internal Revenue Code limit; and plans with other maximum criteria.

The percentage of workers participating in plans with a specified matching percent and the maximum employee contribution matched by employer by selected percentiles; and plans with other matching criteria.

Savings and thrift plans: Method of employer matching contributions
The percentage of workers with a specified matching percent and the specified matching percent by selected percentiles, and other matching methods specified by the employer.

Savings and thrift plans: Maximum potential employer contribution
The maximum potential employer contribution (in percent) by selected percentiles.
